Clinical pharmacy

Our clinical portfolio is expanding on a frequent basis, helping you to advance your knowledge and skills and deliver medicines optimisation in practice for all sectors of pharmacy. From two new focal points a year to our small group learning for hospital pharmacists – Optimise – this section focuses on clinical pharmacy, diseases and therapeutics.

Public health

The public health agenda is embedded in pharmacy, yet topics such as emergency contraception or stop smoking support are as prevalent as ever. As well as our public health workshops, use this section to access a wide range of resources to assure and maintain your competence, all underpinned by the Declaration of Competence system.

Medicines optimisation in care homes essential skills - residential workshop (Days 1 and 2)

Please note: this event is for pharmacists and pharmacy technicians participating in the Medicines optimisation in care homes training for pharmacy professionals pathway.

The aim of this two day residential workshop is to support pharmacy professionals to develop the knowledge, skills and confidence needed to implement strategies to optimise medicines use in care homes.

Learning objectives

On completion of all aspects of this learning programme you should be able to:

Day 1

  • describe the background to the MOCH programme and explain the benefits and challenges for the programme
  • identify poor care, potential medicines related issues and safeguarding issues in care homes
  • outline the importance of leadership in developing the pharmacy professional’s role in care homes
  • describe the context that healthcare professionals work within in a care home
  • articulate what the role of the pharmacy professional undertaking medicines optimisation in care homes involves
  • apply key skills in transfer of care issues, medicines reconciliation and administration of medicines in the care home setting.

Day 2

  • optimise skill mix in the medication review team
  • prioritise care homes residents for medication review
  • undertake person-centred medication reviews
  • advise on the need for low-dose antipsychotics for people with dementia
  • assess frailty and the safety and clinical appropriateness of medicines in frail older people
  • recognise malnutrition in care homes residents
  • deal appropriately with a request to crush tablets in those with swallowing difficulties.

How to use this programme

There is no pre-workshop book associated with this residential workshop. However, you are advised to complete all Units up to and including Unit 6 of the 'Care homes: medicines optimisation essentials for pharmacy professionals' e-course before attending the workshop. At the workshop your facilitator will provide any materials for you to refer to and complete during the event.
