Robert Francis QC's report into failings at the Mid Staffordshire NHS Foundation Trust told a story of appalling suffering of many patients, within a culture of secrecy and defensiveness. The Francis Inquiry focused on one organisation, but it highlighted a whole system failure across the NHS. Issues, such as the culture of care and the vital importance of listening to and being open with patients, their families and advocates, apply across the whole health and care system and are relevant to all healthcare professionals. In essence, what is needed is caring and courageous leadership. This means a patient-centred culture and the need to embed and demonstrate core values, attitudes and behavioural indicators of professionalism.

  • describe your responsibilities in relation to the statutory duty of candour
  • discuss how leadership and culture within your organisation or department impact on candour, openness and transparency
  • discuss the barriers to speaking up and raising concerns.
